All approved MPH forms and documents can be found on the Michigan Pizza Hut -RGM Site
If you have previously saved these documents to your local computer please deleted them and begin using the new forms.
The following forms are the biggest change effective January 2025. Hourly Employee Vacation Request, RGM Vacation Request, Employee Rate Change Form, and Employee Transfer Request Form. These forms no longer open in office and require you to save and download before sending them to Payroll. The new forms start with a link and automatically move to the ASL for approval and then to Payroll for final processing.
Hourly Employee Vacation Request
Hourly employee vacation request forms should be submitted before the close of the payroll date the vacation request is for. Once you filled in all required information, press submit and this request form has been submitted to Payroll for processing.
RGM Vacation Request
The RGM vacation request form works similarly to the hourly employee vacation request form. This form needs to be submitted before the close of the payroll date the vacation request is for.
Enter all information needed paying close attention in line 7 to enter your current ASL.
Once you submit your form it is automatically sent to your ASL for approval. If your ASL rejects your vacation request, you will receive and email notification with the reason and a link to submit a new request, if applicable. When your ASL approves the request it is automatically sent to Payroll for processing.
Employee Rate Change Form

Rate changes require the approval of your ASL before payroll processes the changes. Submitting this form does not automatically update the employees wages in FMS and in Hot schedules. You will need to do that after your ASL approves and at the start of the correct pay period.
When entering information into this form, you should spell the name exactly as it is in FMS and Hot Schedules. Use your MPH store #.
Rate Effective Date
The rate effective date must be a future pay period start date. If you are late to enter a rate change you need to reach out to your ASL and make arrangements with Payroll to correct the late request.
Current Rate of Pay
Just like it is listed in the employee file on FMS, there are 3 wage rate options. Please list these out in the order they are listed in FMS.
New Rate of Pay
List out the new rate of pay in each of the rate boxes. If an employee only has one rate, enter that into "New Rate 1".
Performance Rating
Only employees with a performance rating of on target or above should be considered for a rate change. If you are submitting a rate change because of a promotion you and just mark "On Target" and write promotion in the comments.
Comments
This box is used for important details. If this was a cook and now they are being promoted to Shift Manager, your comment should state that so Payroll knows how to process the rate change.
Select your Above Store Leader
Pay close attention to select your current Above Store Leader. This is an important step to ensure the form is sent to the correct ASL for approval.
ASL Comment & ASL Status
You should not enter any information in this section. This section is held for the ASL approval process.

Check this box if you want a record of your form sent to your email inbox. You do not need to keep this email or forward it to anyone. This is for your record keeping only.
Submit

Employee Request for Transfer Form
Request to transfer employment with Michigan Pizza Hut to another store. Sending and receiving store managers are in agreement with the transfer and the current store ASL is aware and ready to approve the request. Employee transfers should be in place ahead of the next beginning pay period.
When entering information into this form, you should spell the name exactly as it is in FMS and Hot Schedules. This includes the Current Rates 1 - 3.
The effective start date has to be a future date and submitted prior to the start of that payroll.
Select your Above Store Leader
Pay close attention to select your current Above Store Leader. This is an important step to ensure the form is sent to the correct ASL for approval.
ASL Comment & ASL Status
You should not enter any information in this section. This section is held for the ASL approval process.

Additional Forms
Additional forms take you to all the other fillable PDF forms. These forms should not be downloaded to your computer. Rather, always got to the Michigan Pizza Hut -RGM Site to access the forms. This section does have links to the submission forms for vacation requests, rate change requests and employee transfer requests. They are the same forms explained in the article above.
Because the fillable PDF forms contains sensitive information, the document should be filled out and then printed. DO NOT save the document to your computer after filling it out. After printing prepare to send the document in with your next weekly books to the MPH office.
